AFMC 2012 PAID BENEFITS
HEALTH INSURANCE
AFMC pays 80 percent of health insurance premiums for our employees, their spouses and their eligible dependents, including dependents to age 26. Employees are insured through AFMC Employee Funded Plan. The plan features a $25 PCP and $35 specialty copay, $500 individual and $1,000 family deductible, prescription drug benefits, $2,000 out-of-pocket maximum, and 100 percent payment for childhood immunizations. Coverage is effective the first of the month following the date of hire.
AFMC will pay 80 percent of health insurance premiums for employees who participate in our Health Risk Assessment (HRA). The HRA includes a paper survey on employee’s general health, finger-prick lipid panel for cholesterol, waist measurement and blood pressure reading. These tests will be conducted by employee’s primary care physician within 30 days from the date of hire. AFMC will pay 60% of health premiums for employees that do not participate in the HRA. The coverage is for employees, their spouses and their eligible dependents, including dependents to age 26. Employees are insured through AFMC Employee Funded Plan. DENTAL INSURANCE
AFMC pays 80 percent of dental insurance premiums for our employees, their spouses and their eligible dependents. Employees are insured through AFMC Employee Funded Plan. The plan features a $50 deductible per family member, 100 percent of diagnostic and preventive services, 80 percent basic restorative, 50 percent per schedule for major restorative, and a $1,500 annual maximum. Orthodontic coverage is limited to 50 percent payment of eligible charges with a $1,500 lifetime maximum. Coverage is effective the first of the month following the date of hire.
PENSION PLAN
All employees receive a safe-harbor contribution of 3 percent of earnings to the company 401K pension plan, beginning at date of employment. Full-time employees who are at least 21 years of age become eligible for an additional 1 percent contribution on the first of the month following their one-year anniversary. The safe-harbor contribution is immediately 100 percent vested (owned by the employee) and the additional 1 percent contribution vests on a five-year schedule. In addition, employees are eligible to make tax-deferred contributions to the plan beginning at their date of employment.
LIFE INSURANCE
AFMC pays for a life insurance policy equal to 1.5 times an employee's basic annual earnings. Spouse coverage is $5,000. Child coverage for dependents 14 days to 6 months old, is $1,000; coverage for dependents 6 months to 19 years old (25 if full-time student) is $5,000. Coverage is effective the first of the month following date of hire.
SHORT-TERM DISABILITY
This benefit covers only AFMC employees and is subject to a 14-day elimination period. Payment is limited to 60 percent of weekly basic earnings up to a $1,500 weekly maximum for up to 24 weeks. Coverage is effective the first of the next month after the first day of employment.
LONG-TERM DISABILITY
This benefit covers only AFMC employees and is subject to a 180-day elimination period. Benefits are payable at 60 percent of semi-monthly basic earnings reduced by other income subject to benefit maximums. Coverage is effective the first of the month following the first day of employment.
PTO (PAID TIME OFF)
Full-time employees receive 16 days of PTO after completion of the introductory period. (This increases after five years of service.) Part-time employees receive pro rate of PTO leave based on their FTE after completion of the introductory period.
EIB (EXTENDED ILLNESS BANK)
Full-time employees receive eight days of extended illness leave after completion of the introductory period. Part-time employees receive pro rate of extended illness leave based on their FTE after completion of the introductory period.
